Excess skin on the upper eyelids creates a heavy, tired appearance; under-eye bags and hollows on the lower eyelids add years and suggest fatigue that doesn't reflect how you feel. Dr. Fern performs upper, lower, and combined blepharoplasty with meticulous attention to preserving the natural character of the eye — a hallmark that distinguishes his results from the wide, artificial look of less experienced hands.

Are You a Good Candidate?

Ideal candidates for eyelid surgery typically present with one or more of the following:

  • Upper eyelid skin that hangs over the lash line or impairs vision
  • Puffy or baggy lower eyelids with excess fat or skin
  • Persistent under-eye shadows unresponsive to topicals or fillers
  • Tired appearance that doesn't reflect your energy level
  • No significant dry eye issues (to be confirmed at consultation)

The Procedure

Dr. Fern's approach to eyelid surgery proceeds through the following steps:

  • Upper lid: incision placed in the natural crease; excess skin and fat conservatively removed
  • Lower lid: transconjunctival approach (no external scar) for fat redistribution or removal
  • Lower lid skin-only approach for patients with significant excess skin
  • Fat transferred rather than discarded in many cases to avoid hollow appearance
  • Closure with fine absorbable sutures; cold compresses applied immediately

Recovery

Most patients look presentable within 10–14 days. Bruising and swelling peak at days 3–4 and resolve quickly thereafter. Upper lid sutures are removed at day 5–7. Makeup can typically be worn at 10–14 days. Final eyelid contour settles over 2–3 months.

Frequently Asked Questions

Can blepharoplasty be covered by insurance?

Upper blepharoplasty may be covered if excess skin is documented to impair your visual field — a test performed in our office. Lower eyelid surgery is considered cosmetic and not typically covered.

Will I look like myself afterward?

Yes. Dr. Fern's approach is conservative: remove only what needs to be removed, preserve fat where appropriate, and respect the natural anatomy of each eye. Operated eyes that look wide or artificial are the result of over-resection.

Can fillers address my under-eye bags instead?

In some cases, yes — tear trough filler can reduce the appearance of hollowing. Dr. Fern will tell you honestly at consultation whether surgery or filler is the better option for your anatomy.
