Overview

Rhinoplasty is among the most technically demanding procedures in plastic surgery — the nose is the defining center of the face, and changes of even a millimeter matter. Dr. Fern has performed rhinoplasty for more than two decades, developing a refined approach that honors each patient's ethnic background and facial proportions. He is equally experienced in both open and closed techniques and in revision rhinoplasty for patients dissatisfied with prior surgeries.

Are You a Good Candidate?

Ideal candidates for rhinoplasty typically present with one or more of the following:

  • Nasal tip that is wide, bulbous, or drooping
  • Bridge hump or bump that disrupts your profile
  • Nasal asymmetry from genetics or prior injury
  • Breathing difficulty related to deviated septum (functional rhinoplasty)
  • Dissatisfaction with a previous rhinoplasty
  • Facial growth complete (typically 16+ for women, 17+ for men)

The Procedure

Dr. Fern's approach to rhinoplasty proceeds through the following steps:

  • Detailed photography and computer imaging to visualize potential outcomes
  • Open or closed approach selected based on your anatomy and goals
  • Cartilage and bone work to refine shape, projection, and symmetry
  • Septal correction if breathing improvement is a goal
  • Suture techniques to refine the tip without over-resecting cartilage
  • Splint placed externally; internal packing rarely needed with Dr. Fern's technique

Recovery

Swelling is most visible in the first two weeks, during which most patients take time off work. The splint is removed at 7–10 days. By 3–4 weeks, you're presentable in most social settings. However, rhinoplasty requires patience — the nose continues to refine for a full 12 months, and subtle changes occur throughout this period.

Frequently Asked Questions

Open vs. closed rhinoplasty — which is right for me?

The open approach provides greater visibility for complex reshaping. The closed approach leaves no external scar. Dr. Fern chooses the technique based on your anatomy and goals — not a blanket preference.

Can my breathing be fixed at the same time?

Yes. Septoplasty for a deviated septum and turbinate reduction can be performed simultaneously, often with insurance covering the functional portion.

What makes revision rhinoplasty different?

Revision rhinoplasty operates on scarred tissue with potentially compromised cartilage supply. It requires harvesting additional cartilage (often from the ear or rib) and significantly more experience. Dr. Fern accepts revision cases that other surgeons decline.

Will I be able to see results before surgery?

Dr. Fern uses digital imaging at every consultation to simulate potential outcomes. These are guides, not guarantees — but they're an essential part of aligning expectations.
