Gynecomastia: The Plastic Surgery Procedure Men Don't Talk About — But Should

Gynecomastia — the development of breast tissue in men — is far more common than most people realize. Studies estimate that between 40 and 60 percent of men experience it to some degree during their lifetime, with prevalence peaking during puberty, middle age, and older adulthood. Despite its prevalence, it remains one of the most underdiscussed concerns in plastic surgery.

What Causes Gynecomastia

True gynecomastia involves the growth of actual glandular breast tissue — not just fat — in the male chest. It can be caused by hormonal imbalance (elevated estrogen relative to testosterone), medications (certain antidepressants, anti-androgens, anabolic steroids, and others), or most commonly, idiopathic causes where no specific trigger is identified.

Pseudogynecomastia — fatty chest enlargement without glandular tissue — is more common in overweight patients and may respond partially to weight loss and liposuction alone. True gynecomastia does not respond to diet or exercise, because the tissue that needs to be removed is glandular, not fatty.

What Surgery Involves

Gynecomastia correction typically involves two components: liposuction to remove the fatty component of chest fullness, and direct excision through a small periareolar incision to remove the glandular disc of tissue beneath the nipple.

Surgery takes approximately 1.5–2.5 hours and is performed under IV sedation or general anesthesia. Recovery involves wearing a compression vest for 4–6 weeks and avoiding upper body exercise for 4–6 weeks. The majority of patients return to office work within 5–7 days.

Why Men Wait

The most common reason men delay seeking treatment is embarrassment about the condition — an irony, given that the treatment itself alleviates that embarrassment. Many patients have lived with gynecomastia for decades, avoided situations where they needed to remove their shirt, and felt restricted by clothing choices, all without realizing that a straightforward outpatient procedure could address the problem permanently.
